💡 Real life example: Patient comes with fever and weakness. History: Flu since 3 days, diabetic, no medications. Vitals: Temp 102°F, pulse 110, BP 100/60. Look: Pale, sweaty, tired. Labs: WBC high, sugar high.

Tip: Think of assessment as “gathering all the clues”. If something is missed, the care plan can go wrong.

2️⃣ Nursing Diagnosis – Identifying the Problem

After assessment, ask: “What is the patient’s main problem?”

📝 Use nursing terms (NANDA)

✅ Choose the problem that the nurse can solve, not the disease

4️⃣ Implementation – Taking care

Now put the care plan into action.

🛁 Help with hygiene, comfort, movement

🥗 Provide nutrition support

📖 Educate patient and family
