Laryngeal Stridor After Parathyroidectomy
๐ง Why is this the correct answer?
After a parathyroidectomy, the nurse must pay very close attention to the patient’s airway. The parathyroid glands are located in the neck, close to important structures involved in breathing and voice.
If the patient suddenly develops stridor, it means that air is having difficulty passing through the upper airway. This can happen because of swelling, bleeding, or injury involving structures near the larynx.
Stridor is usually a high-pitched, harsh sound heard mainly during inspiration. It is not just a normal postoperative symptom. It can be an early sign that the airway is becoming dangerously narrowed.
That is why the nurse should notify the health care provider immediately and treat the situation as an airway emergency.
In simple words: stridor means the patient may be struggling to get air through the upper airway. Breathing problems always come before less urgent postoperative complaints.
๐ฌ Understanding the Important Medical Terms
Parathyroid: The parathyroid glands are usually four small glands located behind the thyroid gland in the neck. They mainly regulate calcium levels in the blood by producing parathyroid hormone (PTH).
The word parathyroid comes from para-, meaning beside or near, and thyroid, referring to the thyroid gland. So, parathyroid literally describes glands located near the thyroid.
Parathyroidectomy: Surgical removal of one or more parathyroid glands. -ectomy comes from Greek ektomฤ, meaning cutting out or removal.
Larynx: The larynx is commonly called the voice box. It is located in the neck between the pharynx and trachea and contains the vocal cords. The word comes from Greek larynx, meaning the upper part of the windpipe or voice box.
Stridor: A high-pitched, noisy breathing sound caused by narrowing or obstruction of the upper airway. The word comes from Latin stridere, meaning to make a harsh or creaking sound.
Airway obstruction: Anything that partially or completely blocks the normal passage of air into and out of the lungs.
โ ๏ธ Why Can Stridor Develop After Neck Surgery?
The surgical area is close to the airway. After surgery, swelling can develop in the tissues around the larynx. A postoperative hematoma, which means a collection of blood under the tissues, can also put pressure on the airway.
In addition, injury or irritation involving the structures around the vocal cords can affect normal airway function.
โ Why Are the Other Options Incorrect?
Stridor indicates possible upper-airway narrowing. After parathyroid or other neck surgery, this can become life-threatening and requires immediate attention.
Abdominal cramps are not the most immediate life-threatening postoperative complication described here. Although calcium disturbances can cause muscle symptoms after parathyroid surgery, abdominal cramps do not indicate an acute airway emergency.
Difficulty urinating can occur after surgery, especially after anesthesia or with certain medications, but it does not indicate an immediate airway emergency.
Some pain around the surgical incision is expected after surgery. Mild to moderate incisional pain by itself is not a sign of a life-threatening complication.
๐ฉบ What Should the Nurse Watch For?
After parathyroid surgery, airway assessment is a major priority. The nurse should watch for:
- Stridor or noisy breathing
- Difficulty breathing
- Difficulty swallowing
- Increasing neck swelling
- Neck or wound hematoma
- Changes in voice or hoarseness
- Restlessness or signs of respiratory distress
The nurse should immediately report new stridor and rapidly assess the patient’s airway, breathing, oxygenation, and overall condition while following the facility’s emergency protocol.
๐ฆด One More Important Point: Calcium
Parathyroid surgery can also affect the body’s calcium balance because the parathyroid glands normally produce parathyroid hormone (PTH).
If calcium levels fall after surgery, the patient may develop symptoms such as tingling around the mouth, numbness, muscle cramps, and muscle spasms. Severe hypocalcemia can become serious.
However, when the question gives stridor, the priority is the airway. Breathing difficulty is an immediate threat and must be addressed urgently.
โNeck surgery + Stridor = Airway danger.โ
If a patient develops noisy, high-pitched breathing after neck surgery, think airway obstruction first.
After thyroid or parathyroid surgery, stridor is never a symptom to ignore. Airway problems take priority over pain, urinary difficulty, or other less urgent postoperative complaints.
