Frequent Urination in the Third Trimester
๐คฐ Why Does This Happen?
During pregnancy, the uterus gradually increases greatly in size because it must accommodate the growing fetus, placenta, and amniotic fluid.
The bladder is a muscular organ that stores urine before it leaves the body. It lies in front of the uterus. As the uterus enlarges, especially during the third trimester, it can press on the bladder.
Because of this pressure, the bladder may feel full even when it contains a relatively small amount of urine. The woman therefore gets the urge to urinate more frequently.
๐ฉบ Understanding the Medical Terms
Third trimester: Pregnancy is divided into three periods called trimesters. The third trimester is the final part of pregnancy, beginning around the 28th week and continuing until birth. Trimester comes from Latin roots meaning a period of three months.
Uterus: The uterus is the muscular reproductive organ where the fetus develops during pregnancy. The word uterus comes from Latin and refers to the womb.
Bladder: The bladder is a hollow muscular organ that stores urine. The word comes from an Old English term related to a swelling or pouch, describing its balloon-like shape when filled.
Urination: Urination means passing urine from the bladder through the urethra to the outside of the body. It is also called micturition. The word micturition comes from Latin micturire, meaning to want to urinate.
๐ฌ What Happens Inside the Body?
Think of the bladder as a small storage bag. Normally, it gradually fills with urine and stretches as it fills.
In late pregnancy, the growing uterus takes up more space and puts mechanical pressure on this storage organ. The bladder cannot comfortably expand as much, so the sensation of fullness appears sooner.
Therefore, the woman may urinate frequently even though the total amount of urine produced each time may be relatively small.
โ Why the Other Options Are Not the Best Answer
Increased fluid intake during pregnancy: Fluid needs can change during pregnancy, but increased fluid intake is not the main explanation for the characteristic frequent urination in the third trimester. The enlarged uterus pressing on the bladder is the more appropriate explanation.
A urinary tract infection is always present: This is incorrect. Pregnancy can cause frequent urination without an infection. However, symptoms such as burning during urination, fever, lower abdominal pain, or foul-smelling urine should raise concern for a urinary tract infection and require evaluation.
Hormonal changes increase renal blood flow: Pregnancy does cause important changes in kidney function and renal blood flow, particularly earlier in pregnancy. These changes can contribute to increased urine production, but in the third trimester, the most direct explanation for frequent urination is pressure from the enlarged uterus on the bladder.
๐ฉโโ๏ธ Nursing Points
- Frequent urination can be a normal finding during pregnancy.
- Encourage adequate fluid intake rather than unnecessarily restricting fluids.
- Advise the woman not to routinely hold urine for long periods.
- Assess for burning, fever, pain, or other symptoms suggesting infection.
- Teach good perineal hygiene to help reduce the risk of urinary infection.
