๐ซ๐ซ Hemoptysis + Rapidly Progressive Glomerulonephritis
B. Nephrotic syndrome
C. Cystitis
D. Pyelonephritis
โ Answer: (A) Goodpasture syndrome
๐ What is Goodpasture Syndrome?
Goodpasture syndrome is an autoimmune disease in which antibodies are directed against the glomerular and pulmonary basement membranes.
Because both the kidneys and lungs are affected, the patient may develop rapidly progressive glomerulonephritis along with pulmonary hemorrhage.

๐ซ Glomerular injury โ RPGN

โ ๏ธ Goodpasture syndrome
๐ซ Why Does Hemoptysis Occur?
The autoimmune antibodies attack the basement membranes of pulmonary alveoli, causing pulmonary capillary damage and bleeding.
Blood entering the alveoli can produce hemoptysis and respiratory symptoms.
๐ซ Why Does Glomerulonephritis Occur?
The same autoimmune process damages the glomerular basement membrane, producing severe glomerular inflammation and injury.
This can result in rapidly progressive glomerulonephritis (RPGN), characterized by rapid deterioration of renal function.
Hemoptysis + rapidly progressive glomerulonephritis = Goodpasture syndrome
โ Why Are the Other Options Wrong?
A. Goodpasture syndrome
โ
Correct. It classically causes pulmonary hemorrhage with rapidly progressive glomerulonephritis due to anti-glomerular basement membrane antibodies.
B. Nephrotic syndrome
โ Characterized mainly by heavy proteinuria, hypoalbuminemia, generalized edema, and hyperlipidemia. Hemoptysis with RPGN is not its classic presentation.
C. Cystitis
โ Cystitis is inflammation/infection of the urinary bladder and commonly causes dysuria, frequency, and urgency. It does not cause pulmonary hemorrhage and RPGN.
D. Pyelonephritis
โ Pyelonephritis is an infection of the kidney, usually involving the renal pelvis and parenchyma. It does not characteristically cause hemoptysis with RPGN.
๐ Quick Comparison
| Condition | Key Feature |
|---|---|
| Goodpasture syndrome | Pulmonary hemorrhage + RPGN |
| Nephrotic syndrome | Heavy proteinuria + edema |
| Cystitis | Dysuria + urinary frequency |
| Pyelonephritis | Fever + flank pain + UTI |
โGoodpasture hits the LUNGS + KIDNEYS.โ ๐ซ + ๐ซ

๐ Exam Tip: When a question combines pulmonary hemorrhage/hemoptysis with rapidly progressive glomerulonephritis, immediately think of Goodpasture syndrome (anti-GBM disease).
