🧠 Priority Nursing Intervention in Stroke

Stroke is a medical emergency. When a patient experiences a stroke, nursing care must follow the principle of ABC – Airway, Breathing, Circulation.

What is the Priority?
The first and most important nursing intervention in a client experiencing stroke is to maintain airway.

Why Airway is Priority?
During a stroke, the patient may have decreased level of consciousness, difficulty swallowing, or loss of gag reflex. This increases the risk of airway obstruction and aspiration.

If airway is not maintained, oxygen supply to the brain decreases further, worsening brain injury. Therefore, ensuring a clear and patent airway comes before all other interventions.

What the nurse should do:

  • Position the patient properly (usually side-lying if unconscious)
  • Check for gag reflex before giving oral intake
  • Suction secretions if necessary
  • Administer oxygen as prescribed

Why other interventions are not first priority?

  • Range of Motion (ROM) exercises are important but done later to prevent contractures.
  • Maintaining tissue perfusion is essential, but airway must be secured first.
  • Nutritional maintenance is considered after the patient is stabilized.

Exam Tip:
In any emergency situation — Always think ABC first.
Stroke priority = Maintain Airway.
